This page is for attaching the following items to your on-line application:

• Spiritual Biography (1st semester students):  Type a one or two page summary of your life.  Include the following: 

a) Home life growing up; b) Testimony of salvation; c) Fruit and growth after being born again; and d) Your goals and vision for the future.

• Transcripts:  Scan your High School Diploma and/or transcripts (1st semester students).  In addition, we need any other transcripts or diplomas that you may have. If you are continuing from another CCBC campus, we need your current class schedule and grades.  Scan all of these documents and attach them to your application during step four of the process.

• Passport Photo: A simple picture of your face and shoulders.  This should look like a driver’s license or passport picture.

• Proof of Health Insurance:  Scan your current Health Insurance card.
